What is the EMSA skills sign-off form and who can approve it?

This form was created by the State EMS Authority as part of the new regulations that went into effect in October 2005. There is a place for authorized instructors to sign-off on each of the skills in the EMT-Basic scope of practice, as verification that the candidate has been tested on the skills in front of an instructor. The ONLY individuals allowed to initial successful completion of skills testing on this form after the student has successfully demonstrated their capabilities in a scheduled EMT-Basic Refresher course are those approved by the local EMS agency (Sierra Sac Valley).
